HSE - accident reporting
This site links you to the HSE accident reporting centre at Caerphilly. Details of an accident may be reported to the HSE on the site or by:

On the internet site you will be able to complete an accident reporting form F2508 and print off a copy for your records. The investigating authority will then make a decision on whetehr the accident will require investigating or not. Following an accident or serious near miss you should undertake a formal post accident investigation and risk assessment. Please inform your insurers of such occurrences.
Notifiable accidents include serious accidents and near misses suchas lift trucks overturning the collapse of scaffolding, employee off work for more than 3 days or more, employee hospitalisation for 24 hours and public hospitalisation.
